Who is Eligible?
Anyone age 10-21 in the 5 county Traverse Bay Intermediate School district and their infants/young children; enrolled or not enrolled in school. All students under 18 must have parental consent for most visits. Under Michigan law, a minor can receive certain confidential services without parental consent. Please call for more information on which services are offered confidentially.

As of January 1st 2012, your child's height, weight, and Body Mass Index (BMI) may be entered into the Michigan Care Improvement Registry (MCIR), the same registry that his or her vaccination history is kept. This will allow your child's healthcare provider(s) to monitor growth over time and address any weight related concerns with you. Only medically authorized persons can view this information about your child. As with all health information, confidentiality laws are strictly adhered to in order to protect privacy. Please feel free to talk with us regarding any questions or concerns. If you wish not to have your child's height and weight entered into MCIR, please contact our office.
