senior financial Assistance Form - heating
under certain circumstances the commission on aging can help seniors with their heating bill when they have received a shut off notice. before filling out this form, please contact the office to see if you would qualify.
Loan Closet Form
the commission on aging accepts donations of walkers, wheelchairs, bath and transfer benches and loans them out to our clients in need for a small fee.
In addition to your completed forms, we also require copies of:
1) proof of residency (official ID or a copy of a utility bill addressed to you)
and
2) proof of income (income tax return, property tax credit, home heating credit) for ALL members of your household.
