INTAKE SHEET

(click above to obtain the form)

 

Mail your completed Intake/Prescreen form to us at your earliest convenience.  Please check mark which service you are interested in receiving.

In addition to your completed Intake/Prescreen form, you will also need to send copies of:

1) proof of residency (official ID or a copy of a utility bill addressed to you)

and

2) proof of income (SSI Statement, income tax return, etc.) for ALL members of your household.
