The Grand Traverse County Health Department is committed to providing professional health services to the entire community.  Some of our programs do have eligibility requirements in order to participate.  Below is a list of our services and their requirements to determine if you qualify.  Some may have an application to fill out, while others just have income and categorical requirement.  Please feel free to call with any questions you may have at 231-995-6111.

Healthy Kids

 
Healthy Kids is a Medicaid health care program for low-income children under age 19 and for Pregnant Women of any age. There is only an income test. There is no monthly premium for Healthy Kids. Most children who are eligible for Healthy Kids are enrolled in a Medicaid health plan. Healthy Kids provides a comprehensive package of health care benefits including vision, dental, and mental health services.

Plan First

Plan First! is a program through MDCH that provides family planning services to Michigan citizens meeting certain eligibility requirements.

Eligibility
Through this waiver, MDCH offers family planning services to women:

  • 19 through 44 years of age
  • Who are not currently Medicaid eligible
  • Who have family income at or below 185% of the federal proverty level (FPL). DHS will determine eligibilty for this program
  • Who reside in Michigan
  • Meet Medicaid citizenship requirements
  • It is estimated that at least 200,000 women may meet this criteria. Beneficiary applications are available at local health departments, local Department of Human Services (DHS) offices and Title X clinics. For more information providers may call 1-800-292-2550.

Covered Services
A woman's coverage will continue for the duration of the waiver as long as the eligibility criteria are met. Program determinations will be completed annually. Family planning services are defined as any medically approved means, including diagnostic evaluation, pharmaceuticals, and supplies, for voluntarily preventing or delaying pregnancy.

There are no patient co-pays for family planning services, supplies or pharmaceuticals. Services covered under this waiver include:

Initial physical exam and health history, including patient education and counseling relating to reproductive health and family planning options;

  • Annual physical examination for reproductive health/family planning purposes, including a pap smear and testing for sexually transmitted infections when indicated;
  • Brief and intermediate follow-up office visit related to family planning;
  • Necessary family planning/reproductive health-related laboratory procedures and diagnostic tests;
  • Contraceptive management, including drugs and supplies;
  • Insertion, implant or injection of contraceptive drugs or devices;
  • Removal of contraceptive devices;
  • Sterilization services and related laboratory services (as long as a properly completed sterilization consent form has been submitted); and
  • Medications required incidental to or as part of a procedure done for family planning purposes.

The Family Planning service benefit will not include coverage of abortions or treatment of infertility.

MOMS Program

Maternity Outpatient Medical Services (MOMS)

The goal of the MOMS program is to provide immediate health coverage for pregnant women. It provides outpatient prenatal coverage only. The MOMS program is available to provide immediate prenatal care while a Medicaid application is pending. Other women who may be eligible for MOMS include:

  • Teens who, because of confidentiality concerns, choose not to apply for Medicaid, and
  • Non-citizens who are only eligible for emergency services only

The woman must use Medicaid benefits if and when they become available. Prenatal health care services will be covered by MOMS and/or Medicaid for the entire pregnancy and for two months after the pregnancy ends. There is an income test for all persons except teens. The local health department can help women apply for the MOMS program.

CHAP CHAP is a free program for qualifying, uninsured residents of Benzie, Grand Traverse and Leelanau counties.  To qualify, you must 1) live in one of the three counties; 2) be 19 years old or older; 3) not have any other medical coverage or insurance; fall within the income and asset guidelines which are not more than:
  • $1,733/month for 1 person
  • $2,333/month for family of 2
  • $2,933/month for family of 3
  • $3,533/month for family of 4
  • $4,133/month for family of 5
  • $4,733/month for family of 6

You must also complete an application for eligibility determination and attend an orientation meeting.  Enrollment sites are:

Northern Dental Plan Sponsored by Dental Clinics North, Northern Dental Plan offers dental services at a reduced fee for people without dental insurance. If you are eligible, you'll receive a FREE initial visit with a participating dentist, including x-rays and an exam, and reduced rates for all treatment following. Membership in the Northern Dental Plan is $50.00 per person.

To be eligible for the program, you must:
1) Have no other dental insurance
2) Fall within the following income guidelines:
  • $20,800/year for 1 person
  • $28,000/year for family of 2
  • $35,200/year for family of 3
  • $42,400/year for family of 4
  • Add $6,360/year for each additional person

3) If someone in your household is a WIC or Medicaid recipient, or is eligible for food stamps, you meet the income requirements.
4) If you have a current MIHealth Card, you are eligible to receive care at Northern Dental Plan reduced fees without paying the membership fee.

WIC

WIC is a special food and nutrition program that promotes good health and nutrition for pregnant & postpartum women, infants and children under 5 years of age. To be eligible, you must:

  • Be pregnant
  • Be up to 6 months postpartum
  • If breastfeeding, can be postpartum up to 1 year
  • Have a child that is under the age of 5

You must also meet income guidelines determined by the State of Michigan.

CSHCS Children's Special Health Care Services (CSHCS) is a program within the Michigan Department of Community Health for children and some adults with special health care needs and their families.  We help persons with chronic health problems by providing:
  • Coverage and referral for specialty services, based on your child's health problems.
  • Family-centered services to support you in your role as the primary caretaker of your child.
  • Community-based services to help you care for your child at home and maintain normal routines.
  • Culturally competent services that demonstrate an awareness of cultural differences.
  • Coordinated services to pull together the services of many providers who work within different agencies.

To join, children must have a qualifying medical condition and be 20 years old or under.  Persons 21 and over with cystic fybrosis or certain blood clotting disorders also may qualify for services.
CSHCS covers more than 2,000 diagnoses.

If you think your child qualifies for CSHCS, ask your specialist to send a report about your or your child's condition to us.  If your child has not been seen by a specialist, we can make arrangements to have an evaluation take place.

Your child's medical condition, not your income, determines if you qualify for this program.  If your child qualifies, he or she will be invited to join the program and an application will be mailed to you. A review of this application will determine if we will ask you to share in the cost of services.

MIHP Maternal Infant Health Program (MIHP) is a program for all Michigan women with Medicaid health insurance who are pregnant and all infants with Medicaid. It is a benefit of their insurance.  MIHP provides support to promote healthy pregnancies, good birth outcomes, and healthy infants.

Services include:
  • Maternal and infant health, psychosocial and nutrition assessment completed by nurse, social worker, or nutritionist
  • Registered Nurse, Licensed Social Worker and Registered Dietician team development of beneficiary care plans
  • Coordination of MIHP services with the beneficiary's medical care provider and Medicaid Health Plan (who assist and supplement that care)
  • Registered Nurse, Licensed Social Worker and Registered Dietitian home or office visits provided with interventions based on the beneficiary's plan of care
  • Transportation services arranged if needed
  • Referrals are made to local community services (e.g., mental health, substance abuse, domestic violence, basic needs assistance) as needed
  • Referral to local childbirth education or parenting classes

Breast & Cervical
Cancer Screening

(BCCCP/WomanCare)

BCCCP is offered at the Grand Traverse County Health Department to qualifying women, including women who:
  • are 40 to 64 years old
  • have limited income
  • do not have HMO insurance or Mediare coverage

Services include:

  • Breast exams
  • Pap tests
  • Mammograms

Reproductive Health Services The Reproductive Health Clinic at the Grand Traverse Health Department serves males and females of reproductive age.

Services include initial and annual exams, Pap smears, pregnancy tests, tests and treatment for sexually transmitted infections and HIV testing. All methods of birth control (pill, condoms, diaphragm, IUD, Depo, patch, ring, etc.) are available.

Medicaid and Plan First! are accepted and charges for others are on a sliding scale. No one will be denied services because of inability to pay.
