The Grand Traverse County Health Department is one of ten locations for Dental Clinics North. Dental health services are provided to children and adults who have Medicaid, Delta Dental (Healthy Kids Dental and MIChild) or *Northern Dental Plan. Services include: • Oral Exam/Cleaning • Fluoride Treatment • Fillings • Education • Other Dental Procedures *NORTHERN DENTAL PLAN Northern Dental Plan offers dental services at a reduced fee for people without dental insurance. If you are eligible, you'll receive a free initial visit with a participating dentist, including x-rays and an exam, and reduced rates for all treatments. Membership in the Northern Dental Plan is $50.00 per person. To be eligible for the program, you must: • Have no other dental insurance • Fall within the following income guidelines: FAMILY SIZE MAXIMUM INCOME 1 $21,660 2 $29,140 3 $36,620 4 $44,100 5 $51,580 ($7,480 for each additional person) If someone in your household is a WIC or Medicaid recipient, or is eligible for food stamps, you meet the income guidelines. If you have a current MIHealth card, you are eligible to receive care at Northern Dental Plan reduced fees without paying the membership fee. Or you can pick on up at any Dental Clinics North locations and at participating health departments and participating private dentist's offices. Or call Dental Clinics North to register with a credit card over the phone at: 877-321-7070. Once your application and membership fee have been received and processed, your card will be mailed to you. You will receive your membership card by mail in 2-3 weeks.
